Healthcare Facility Regulation Division (HFRD) Application Migration
The Georgia Department of Community Health (DCH)'s (HFR) division will be migrating all licensing activities for Private Home Care Providers from the current Georgia MMIS (GAMMIS) system to the new Georgia Healthcare Accreditation and Licensing Electronic System (HFRDGHALES). Please complete and submit all applications before the end of the business day on Friday, November 22, 2024.

Healthcare Facility Regulation Division

The Healthcare Facility Regulation Division (HFRD), a division of the Department of Community Health (DCH) and composed of the Office of Facility Licensure (OFL), licenses and regulates various acute/specialized care, personal care, and long term care facilities in Georgia. Facilities wanting to enroll, renew or make changes with HFRD can easily do so online from the HFRD Application page found within the HFRD menu. This online enrollment system also provides the option to upload electronic attachments against a submitted HFRD application as well as view a user friendly PDF of the submitted application.


To view the status of a submitted HFRD application, this is available on the HFRD Application Status page found within the HFRD menu.
