Healthcare Facility Regulation Division (HFRD) Application Migration
The Georgia Department of Community Health (DCH)'s (HFR) division will be migrating all licensing activities for Private Home Care Providers from the current Georgia MMIS (GAMMIS) system to the new Georgia Healthcare Accreditation and Licensing Electronic System (HFRDGHALES). Please complete and submit all applications before the end of the business day on Friday, November 22, 2024.

HFRD Application Status

This page provides a status for HFRD applications submitted to Gainwell Technologies. Along with a License Tracking Number (LTN), either a Business Name (as submitted on the application) or Facility ID are required to retrieve the status of the application.
