| Field Descriptions: |
| continue application | This button allows the user to search for an existing application using the business or last name and the application tracking number (ATN) assigned after the application was successfully saved. |
| exit | This button is used to exit the panel and return to the enrollment landing page. |
| new application | This button directs the user to the first page in the provider enrollment process to begin a new application. |
